Request the code from your carrier.
Turn the phone on with a SIM from a different carrier inserted.
Type the password when prompted.

If your carrier won't do it, or you don't have a carrier for that phone,
there are services that will do it for you.

Check craigslist. In my area there are several unlocking services. I had
a V195 unlocked recently. It cost $15.

It's not like some Nokia phones where you can generate an unlock code
based on the ESN.
